I consent to having a dental impression or scan taken — in person or via a mailed impression kit — for the purpose of fabricating a custom mouthguard, under the direction of Dr. Jessica Delgado, DDS, MS, a USA board-certified orthodontist.
I understand that a ReeBite mouthguard is a custom-fit performance sports appliance for protection, and is not treatment for sleep apnea, bruxism, or a temporomandibular (jaw-joint) disorder.
I understand my health information is handled under the HIPAA Notice of Privacy Practices and stored on the practice's secure system.
I authorize the charge shown at checkout (first guard $99, renewals $25, with free shipping; an optional additional guard adds $25). Payments are processed securely by Stripe.
I understand my guard is backed by a 30-day fit guarantee as described in the fit-guarantee policy.
